Learn more about HDME (Multidimensional Diagnosis Tool of Exclusion), a tool designed within the project “PACT”: ProActive Case-based Targeted for social inclusion. The European project PACT is an initiative of innovation in social services aimed to test a new model of care for persons at risk of social exclusion in the region of Castilla y Leon, Spain, based in two interconnected strategies: the public-private partnership and social investment that will be piloted with a sample of minimum income beneficiaries.
The project is funded by the European Commission through the Programme for Employment and Social Innovation of the European Commission, “EASI” (2014-2020).
The project is led by Gerencia de Servicios Sociales which is the Public Administration (belonging to the Ministry of Family and Equal Opportunities of the Government in Castilla y León, Spain) responsible of design, planning and management of all social services in the region. The rest of project partners are the Municipalities of Valladolid, Leon and Salamanca, the Provincial Council of Valladolid, the University of Valladolid and EAPN Castilla y Leon, all of them from Spain.
